Instructions to schedule your Covid shot

I want to share what I learned in getting a Covid shot to help you schedule your own appointment. I started with looking up eligibility at the CT.gov portal.

Then I went to the Department of Public Health and registered with VAMS. You will get a confirmation email and a sperate email with a link to login and schedule your appointment. I had a hard time with VAMS so I ended up calling (at the bottom of page) and scheduled over the phone. The appointments are 2-3 weeks out.

Find a Vaccine Provider Near You GO Schedule your COVID-19 vaccine. Get Notified When You're Eligible Stay connected and never miss an alert.

It will then send you to DPH to register with VAMS to book your appointment.


Online VAMS Enrollment (for Eligible Individuals)

Please Note: This form is optional and is only for Requesting enrollment with the Vaccine Administration Management System (VAMS). The CT Department of Public Health will identify which phase you meet for vaccine rollout. Once you meet the current phase, then you will receive an email from VAMS to complete your registration and schedule an appointment for COVID-19 vaccination. You do not need to register with VAMS if you have other options for getting the vaccine.   • For Eligibility for Education and Childcare Professionals and Staff, please visit: Vaccine Eligibility for Education and Childcare (ct.gov)

    • Prior to registering here: Education and childcare workers should talk with their employers to ensure they are not already scheduled into a dedicated clinic.

If you are currently eligible to register, please complete this form below to request enrollment in VAMS.

You will need to enter your full name, date of birth, eligible occupation or Age eligibility, Zip Code and email address. The information you enter will only be used for purposes of scheduling your vaccination, and will be kept private.

  • You will receive a confirmation number after you have submitted your enrollment. Then, within 24-48 hours, you will receive an email from VAMS "no-reply@mail.vams.cdc.gov" or "no-reply@envelope.mail.vams.cdc.gov" with a link to complete your registration and schedule your appointment for COVID-19 vaccination.

Schedule Your COVID-19 Vaccine

The state offers residents and workers several options for scheduling their COVID-19 vaccine appointment.


Option 1: Search for a Vaccine Site Enter your address or ZIP Code to view all the locations nearest you. You can also search for a specific provider to view associated locations. GO

Option 2: Schedule by Phone Prefer to schedule by phone? Select sites offer appointment scheduling via phone. Due to high volume, hold times may vary. Please note that some vaccine sites allow phone scheduling through a direct clinic phone number. Search by location to view additional sites and scheduling options.
